Anyone have these? I just finished filling mine out a couple days ago because it was recommended on my hospital pre-admittance form. Probably should have had them for my previous pregnancy but better late than never. The durable power of attorney form was easy - just listing your first and second choice persons with their contact info. The advance directives were little more involved, you have to be more specific about what you desire to happen. Mine had options to initial what you wanted but you could strike out parts or add more as you wish, then you have two witnesses (non-relatives and not your healthcare providers) sign.
