Birth Certificate Questions...

The hospital gave us the Birth Certificate form to fill out so it will be all done when we get to the hospital and there are some weird questions on there that I do not remember having to answer for DS.

Such as:
1. the mother and father's education level
2. how much the mother weighed before pregnancy and how much the mother weighed after birth
3. Did the mother smoke before and/or during pregnancy and if so, how much
4. Did the mother drink before and/or during pregnancy and is so, how much

Why in the world would they need to know this stuff???? I don't mind answering, but these questions just seem bizarre to me. Why can't they just ask for the baby's name, parents name, address, date and time of birth? Sorry, I just needed to vent. I get annoyed with the government invading my privacy, I guess, especially when it comes to my weight! LOL!

That sounds more like an additional questionnaire for research purposes. I wonder if it is from the government or if it's the hospital or another third party requesting the info. That definitely has nothing to do with getting a birth certificate. And, it isn't required to get a birth certificate. Strange indeed!

I would say answer them if you want to but if you don't, then don't. They can't make you.

It's for the TX Vital Statistics and the hospital sends it directly to them. Almost reminds me of a census form! So weird! I already answered them. I just thought that was so strange to ask on a birth certificate form. Why not just send out a separate questionnaire. Makes sense why they ask you to take it home and fill out before coming to the hospital though. It takes almost an hour just to fill out!

yeah, its for statistical purposes Smile i've always done pre-registration with an induction and with my csection so they fill it out for us there. Smile

I don't think these were questions they asked me when I had DS 5 years ago or maybe I was drugged when I answered them. Smile I just remember them coming into the room and asking what we were going to name him, how to spell it, verify our home address, and jotting down the time of his birth.

You are so right, Emma. We lie about our weight anyways! LOL!

I don't remember getting these kinds of questions with DS. I DID get these questions from the hospital though, so maybe it's a hospital form?

On the topic of birth certificates, I am wondering if the mother's maiden name or married name should be listed? On my DS's my married name and thought it should be maiden but maybe it varies from state to state?

The state info is all over the packet stating that you return it to them so I'm thinking this is all from the state. I've already filled out the pre-registration form for the the hospital and some of similar questions were on there. They may collect the info for the hospital and share it with them though. I don't know, but it was very strange having to answer those questions on the same page that they ask the general birth certificate questions. It has to be something new they are doing.

I did have to include my maiden name on the packet and you check off whether you are single, married, divorced, or widowed. Then you include the fathers information.

I think both are on my girls birth certificates. I think it has a place for maiden name and married name...Not certain though. I know both were asked and I checked the spelling on them though.

I wouldn't answer those questions. None of their business. Wouldn't answer them for the birth certificate and wouldn't answer them for pre-registration at the hospital either.

On DD's birth certificate, I had to list my full maiden name (as if I wasn't married).

That is weird they're asking your weight! But I think they asked about my education level and such as well. But I don't really remember to be honest. They had me list my maiden name as well as my married name too.

Just my married name is on the certificate and not my maiden with DS, so when my name changes after I get remarried, it's going to be kinda tacky which is why I thought the maiden name should be on there too. I do live in IN so maybe that explains it. I will have to check first before I fill out this LO's birth certificate.

My maiden name is on DD's birth certificate and my mom's maiden name is on my birth certificate and DH's mom's maiden name is on his birth certificate... I think having the mother's maiden name is fairly common. (That covers three states: Alabama, Georgia, and North Carolina)

I agree! That is why I think mine is wrong. I know I filled the birth certificate out right. I just think it was a mistake and I might have to go back and get it fixed. I'll see what they do with this one... maybe I won't even list my married name this time.
